1. How do I schedule for a test?

Visit www.hdmaster.com to print out the 1101 & 1402 application forms and the testing schedule. Or you may request a testing packet by calling 877-201-0758. You need to send the application to D&S DT, PO Box 418, Findlay, OH 45840. Send your payment and a copy of your training registration roster (prior to 10/1/09)/training certificate (10/1/09 and after) or challenge approval letter from the State.

 
2 .What do I need to take to testing?

You must take a government issued non-expired, signed, photo ID and your social security card. If you arrive without these 2 forms of identification, you will not be permitted to test and you will be considered a no show.

 
3. How do I reschedule my test?

If your test is more than 24 business hours, you may call and get one free reschedule over the phone. If you have used your free reschedule you will be required to pay the $35.00 reschedule fee prior to rescheduling. If your test is in less than 24 business hours, you will not able reschedule.

 
4.How do I cancel my test?

You need to put your request to cancel in writing and send it to D&S so that it is received at least 24 business hours prior to your test. If we receive it more than 24 business hours prior to the test, you will receive a refund minus the $25.00 cancellation fee if you were a self pay. If you were TDH funded, the facility will be billed the $25.00 cancellation fee.

 
5. How do I challenge the CNA test in Tennessee?

Visit www.hdmaster.com and print off the challenge application. You may also call the Tennessee Dept. of Health at 615-741-7173.

 
6. How do I renew my license?

Certified Nurse Aides are required to re-certify every two years and must provide verification of having worked at least one 8 hour shift during the past 24 months in a state licenses or certified facility. Please contact the Nurse Aide Registry with nay questions at 615-532-5171 or 800-778-4504.

 
7. How do I get my CNA test results?

You may access your test results online at www.hdmaster.com. You will need to enter your social security number, test date and date of birth. You will also receive your test results via mail or email 5-7 business days after your test date.

 
8. What score do I need to pass the test?

You must score at least 75% to pass the written test. You must score at least 80% on each skill task and not miss any key steps (bold print in the candidate handbook.)

 
9. Do you have practice tests available?

You may purchase a 100 question practice written test at www.hdmaster.com.

 
10. I only have my training certificate, no registration roster. Can I use that instead?

If you completed training prior to 10/01/09
No, the training program is required to provide you with a registration roster for testing. If you are unable to obtain a registration roster, please contact Wanda King at 615-532-5171.

If you completed training on/after 10/01/09
You will need to submit the state mandated training certifiate, the state mandated certificate has the seal of the TN Dept of Health at the top. Training rosters with completion dates of 10/01/09 or after will not be accepted.


 
11. I was certified in another state & want to transfer my license to Tennessee. How do I do that?

You need to contact the Nurse Aide Registry at 800-778-4504 or 614-741-7173 to see if you are eligible for reciprocity.


 
12. Where can I take CNA training classes?

You may contact the Tennessee Dept. of Health at 615-532-5171 for a list of training programs in your area.
