1. How do I schedule for a test?

Visit www.hdmaster.com to print out the 1101 & 1402 application forms and the testing schedule. Or you may request the handbook and forms by calling 877-201-0758. You need to send the application to D&S DT, PO Box 418, Findlay, OH 45840. Send your payment and a copy of your training certificate or challenge approval letter from the State.
You may also use your individual login information to access your record through the Students/Candidates button. Once you enter your record you will be required to read the information at the bottom of the screen and place a check mark in the attestation box. After you have completed that step, you will be able to click on the "Prepay by Credit Card" to make your testing payment. Once the payment has been made, you will be able to select the test site and date that you would like to test. When you have made your selection, click on the submit updates button to save the selected date and time. You will see a test confirmation on your computer screen if the scheduling was successful.

2 .What do I need to take to testing?

You must take a government issued non-expired, signed, photo ID and your social security card. If you arrive without these 2 forms of identification, you will not be permitted to test and you will be considered a no show. The name entered into the database for you must match exactly the name printed on your government issued ID and social security card. If the names do not match exaclty, you will be considered a no show.
